CNL Benefit Plans
Regular full-time associates are eligible for CNL's benefit plans from date of hire. Costs for coverage (where applicable) will be discussed by Human Capital.
Medical/Hospitalization/Prescription Coverage
United Healthcare is the carrier for the medical benefits.
United Healthcare offers two Point-of-Service (POS) plans; both provide you with the freedom to see any physician or healthcare professional from the network, including specialists, without a referral. Co-pays for office visits are $20, $100 emergency room co-pay, and 90% or 100% (depending on your plan choice) benefit for in-patient hospitalization and diagnostic services. There is a $250 individual annual deductible in-network and a $500 individual annual deductible for out of network benefits under both POS plans.
Dental Coverage
MetLife is the carrier for dental benefits.
Preventive Care coverage is reimbursed at 100% (of reasonable & customary fees) with deductible waived. This consists of one visit every six months for oral examinations, dental cleaning and bite-wing x-rays. Other services are covered at 80% or 50% with a $50.00 per person annual deductible and a maximum benefit of $1,500 per calendar year.
Short-Term/Long-Term Disability Insurance
Reliance Standard is the provider for STD & LTD coverage.
The Short-Term Disability (STD) coverage provides a 60% (of weekly earnings) benefit upon the associate's disability due to accident or illness. The benefit is available for up to 12 weeks after an 8 day elimination period. The Long-Term Disability (LTD) coverage provides a 60% (of monthly earnings, up to $15,000) upon disability continuing past 12 weeks, up to age 65. The basic STD & LTD coverage is provided by the company. Additional supplemental coverage is also available for both the STD and LTD insurance at the associate's expense.
Long-Term Care Insurance
UNUM is the provider for the Long-Term Care coverage.
Long-Term Care provides an associate with $1,000 per month toward the cost of certain care services. The basic Long-Term Care Insurance is provided by the company. Additional supplemental coverage is available for both the associate and certain family members at the associate's expense.
Life Insurance
Reliance Standard is the carrier for the (group) life insurance benefit.
CNL provides a basic life insurance benefit to all regular full-time associates at no additional cost. For further information on Life Insurance and Additional Supplemental life insurance, please consult with Human Capital.
Vision Insurance
VSP is the carrier for the voluntary vision benefit.
The voluntary vision insurance plan provides an eye exam and prescription glasses with lenses covered in full every 12 months, or an allowance of $120 is given to apply to the cost of contacts and the fitting and evaluation exam. Flexible Spending Accounts
FSA accounts are administered by Ceridian.
Flexible Spending Accounts (FSA) help you set aside pre-tax money to pay for eligible health care and dependent care expenses. The amount that you choose to put into an FSA is divided by the number of pay periods you will have during the plan year. This amount is then deducted from your pay throughout the year before taxes are calculated. When you have an eligible expense, you submit a claim form and you are reimbursed. Your reimbursement will never be taxed. For additional information on this benefit, please consult with Human Capital.
Paid Time Off (PTO)
Following a 90-day introductory period, all regular full-time and part-time associates are eligible to participate in the PTO program. (Regular part-time associates participate on a prorated basis based on their regular hours worked.) PTO is used for vacation, illness, or personal time. Up to five days of PTO may be carried forward into the next calendar year in an Extended Leave Bank (ELB) for illness-related absences. PTO must be approved by your supervisor. PTO is granted at the beginning of each year although it is accrued or "earned" each pay cycle worked. Upon leaving the company, adjustments are made to an associate's final pay check either paying out earned but not used PTO or recovering costs from PTO used but not yet earned.
Holiday Pay
Eleven (paid) holidays are observed during the year. Regular part-time associates are eligible to receive a pro-rata share of this benefit based on average number of hours worked.
Birthday Holiday
As a part of our vision to be the preferred place to work, all regular full-time or part-time associates are eligible, upon completion of the 90-day introductory period, to take their birthday off with pay. The new birthday holiday will give associates an additional day off with pay as a special and unique day to show CNL’s appreciation for our associates.
401(K) Plan
CNL's 401(k) Plan is administered by Fidelity Investments. All associates (with the exception of those titled Intern) are eligible to participate in the 401(k) Plan at the beginning of the month following their first full payroll cycle:
- Associates are allowed to defer a portion of their wages (pre-tax) via convenient payroll reductions (1-90% of annual salary not to exceed $15,500 annually).
- CNL offers 27 funds in which you can direct investments. You can complete your investment allocation, as well as check your account balances, via the Fidelity website (http://www.401k.com).
- CNL will match 50% of the first 7% of the deferred wages up to eligible maximums after six months of employment.
- Each associate may visit http://www.401k.com the Fidelity website for investment information. Vesting schedules and enrollment guidelines will be sent to all associates.
Educational Assistance
On a limited basis, CNL may reimburse the cost of tuition for approved courses that are degree seeking or directly related to your job provided you successfully complete the course with a "C" or above. Regular full-time associates are eligible to participate after the completion of 90 calendar days of employment. Reimbursement (including tuition and books) is limited to a calendar year maximum of $3,000.
Employee Assistance Program
All associates may receive confidential professional assistance in resolving personal, work related, and family matters through Horizon Health. This service is provided at no charge, and is available to associates 24 hours a day, and may be used by any member of the associates' household.
Adoption Assistance Program
The Adoption Assistance Program is offered to eligible associates to help offset some of the expense incurred during the adoption process. Reimbursement of specific expenses is limited to a calendar year maximum of $2000.
